The study, comparing 228 pregnant women prozac and pregnancy who took the drug with 258 controls who did not, showed an increased incidence of three or more minor prozac and pregnancy structural anomalies in babies exposed during the first trimester. It also showed an increased risk of premature delivery and poor perinatal condition in those exposed during the third trimester. "These data suggest that if pregnant women are unable to discontinue fluoxetine prozac and pregnancy therapy before the third trimester, the risk of perinatal prozac and pregnancy complications is increased," said the prozac and pregnancy investigators from the division of dysmorphology and teratology in the pediatrics department of the University of California-San Diego. ‘Seems unjustified’ However, in an editorial accompanying the report in prozac and pregnancy The New England Journal of Medicine, Dr. prozac and pregnancy Elisabeth Robert of the Institut Européen des Génomutations in Lyon, France, said, "It seems unjustified to use these prozac and pregnancy new results as a reason to withhold fluoxetine from women who require an antidepressant drug during pregnancy." She took exception to the researchers' methods and interpretation of results, and noted the findings are "in contrast to ... five cohort studies, which included approximately 450 pregnancies prozac and pregnancy and which focused on the relation between fluoxetine and developmental effects, (and) prozac and pregnancy suggested that children exposed in utero, whether early or late in gestation, do not have an increased risk of birth defects, poor perinatal condition, or neurodevelopmental delay." Those prozac and pregnancy studies include two reports from the Motherisk program at Toronto's Hospital for Sick Children. Dr. prozac and pregnancy Gideon Koren, director of the program and professor of pediatrics and pharmacology at the University of Toronto, discussed the group's fluoxetine studies in a recent interview with The Medical prozac and pregnancy Post. (See The Medical Post, Feb. 20, 1996.) Women in the new study were drawn from those who had called the California Teratogen Information Service and Clinical prozac and pregnancy Research Program asking about prozac and pregnancy the potential teratogenic effects of fluoxetine. Controls prozac and pregnancy were taken from callers not taking the drug and whose exposures were prozac and pregnancy deemed not to be teratogenic. Pregnancy outcomes of women in the fluoxetine group were analysed separately for those women who took the prozac and pregnancy drug during only the first trimester and those prozac and pregnancy who took it later in pregnancy. For 97 infants born to women in the early-exposed group, there was a higher incidence of babies having three or more minor structural anomalies (15.5%) than in 153 control prozac and pregnancy infants (6.5%). The researchers, however, did not indicate what the anomalies were, but defined them as structural defects that have no cosmetic or functional importance and that are known to occur in less prozac and pregnancy than 4% of the general prozac and pregnancy population. The finding, however, "indicates that fluoxetine therapy during the first trimester of pregnancy has an effect on embryonic development," prozac and pregnancy they said, and "raises the possibility of an associated defect in the development of the central nervous system that may become evident when the infant is older." The 73 infants who were exposed during the third trimester had higher rates of premature delivery, admission to special care nurseries and poor neonatal adaptation, including respiratory difficulty, cyanosis on feeding prozac and pregnancy and jitteriness, compared to either the early-exposed infants or controls. Lower birth weight Their birth weight was also lower and birth length was shorter, said the researchers. In the editorial, Dr. Robert pointed out several methodological factors that could have contributed to the findings. First, she suggested the fact that the control group was made up of women who were presumably not depressed didn't allow for the possibility that the illness itself was a confounding factor in the observations. Several outcomes -- prozac and pregnancy prematurity, admission prozac and pregnancy to a special-care nursery and poor neonatal adaptation prozac and pregnancy -- are known to be associated with psychiatric disorders in the mother, she said. Maternal age was higher in the fluoxetine group, which may partly explain prozac and pregnancy the observed prozac and pregnancy excess of poor perinatal outcomes, she added. In addition, 30% of the fluoxetine-treated prozac and pregnancy mothers were taking other psychoactive drugs. "It is important for physicians who take care of pregnant depressed women not be confused by these contradictory results," Dr. Robert said, referring to the other studies showing no adverse effects from taking fluoxetine in pregnancy. "Although several hundred women have taken fluoxetine during pregnancy without obvious reproductive or developmental toxic effects, its safety for pregnant women and their fetuses has yet to be thoroughly assessed," she added. "I do not think that fluoxetine or tricyclic antidepressant drugs have been clearly prozac and pregnancy proved unsafe for pregnant prozac and pregnancy women, but their use involves a calculated risk because of their uncertain side effects." Researchers have examined the effects of Prozac (commonly used to treat depression and other problems) in more than 1,000 pregnant women and found that those women were no more likely to give birth to a baby with a major birth defect than other women. While one study did prozac and pregnancy show a slightly greater risk of miscarriage among women on the drug, many others have not.It's also unlikely that your baby will suffer from withdrawal symptoms if you use Prozac during your pregnancy. Although some newborns of mothers on Prozac have shown symptoms such as irritability, shaking, or increased crying, most have not had these problems. Mothers who take Prozac during the third trimester of pregnancy risk premature delivery, the report says. They also put their infants at risk of "poor neonatal adaptation," including: Jitteriness Quick, shallow breathing Low blood sugar Low body temperature Poor muscle tone Trouble breathing Weak or absent cry Diminished reaction to pain Not prozac and pregnancy getting enough oxygen while feeding Increased admission to special-care nurseries Prozac taken by breastfeeding mothers, the report says, may retard prozac and pregnancy infants' early growth. These aren't Prozac's only effects on reproduction. The report also says that the antidepressant also has "reproductive toxicity." Adults taking the drug may find sexual orgasm difficult or prozac and pregnancy impossible. And some women taking the drug may have changes in the length of their menstrual cycle. The report comes from the Center for prozac and pregnancy the Evaluation of Risks to Human Reproduction (CERHR). CERHR is part of the federal National Toxicology Program at the National Institutes prozac and pregnancy of Health. Its job is to convene expert panels to look at possible threats to human reproduction -- including child development. The chairman of the 12-member Prozac panel was pediatrician and pharmacologist Ronald N. Hines, PhD, co-chief of pediatric pharmacology, pharmacogenetics, and teratology at Medical College of Wisconsin, Milwaukee. "Patients should be aware that these are the risks of taking Prozac," Hines tells WebMD. "They really should prozac and pregnancy talk with their doctors about them before going on the drug." Tarra Ryker, spokeswoman for Prozac maker Eli Lilly & Co., says that the risks of untreated depression far outweigh those of prozac and pregnancy properly prescribed antidepressants. Lilly prozac and pregnancy is a WebMD sponsor. "We don't believe there is sufficient evidence to conclude that Prozac is a developmental or a reproductive toxin," Ryker tells WebMD. Prozac Benefits -- Even During Pregnancy -- May Outweigh prozac and pregnancy Risks "There may be some side effects that cannot be avoided, but the benefits of this drug may be so great they outweigh the risks," Hines says. And prozac and pregnancy the benefits may be huge, says psychiatrist Mark I. Levy, prozac and pregnancy MD, assistant clinical professor at the University of California, San Francisco. "The main benefit is it is preventing depression at a vulnerable time in a vulnerable person," Levy prozac and pregnancy tells WebMD. "Prozac can prozac and pregnancy be helpful in warding off postnatal depression, which is bad for a woman and bad for her baby." Women suffering from depression should not stop taking Prozac or other antidepressants, says Lee S. Cohen, MD, director of the perinatal and reproductive psychiatry clinical research program at Massachusetts General Hospital and Harvard Medical School. Cohen serves as a consultant to Prozac maker Eli prozac and pregnancy Lilly & Co. He testified at the CERHR panel's public hearing. "If women stop antidepressant treatment during pregnancy, their risk for recurrence of their prozac and pregnancy underlying depression is very, very high," Cohen tells WebMD. "And depression during pregnancy is associated with bad outcomes for newborns and very bad outcomes for women." Hines, Cohen, Levy, and Ryker agree that pregnant women -- or women planning to become pregnant -- should not stop taking Prozac or other antidepressants until they've talked it over with their prozac and pregnancy doctors. "Women should not be overly prozac and pregnancy alarmed about this report," Cohen says. "It describes a series of prozac and pregnancy symptoms that are very difficult to ascribe prozac and pregnancy either to Prozac or to any other commonly used antidepressant taken before, during, or after pregnancy." Hines disagrees. While he notes that there is no "hard and fast" prozac and pregnancy proof that Prozac is the definite cause of the toxicities the panel describes, prozac and pregnancy the panel members find the evidence disturbing enough to recommend caution. "We think that there is some concern," Hines prozac and pregnancy says. "We don't prozac and pregnancy think these toxicities are life threatening, but prozac and pregnancy we feel that to be one hundred percent confident in the safety of this drug, it would be good to have more studies done." The panel concluded its report by recommending that more human and animal studies look closely at the effects of Prozac and other antidepressants on reproduction and child development. At least one new study will soon arrive. At next week's meeting of the American Psychiatric Association, Cohen will report findings from a study of 200 women with clinical depression who stayed on or stopped taking their antidepressants during pregnancy. "We found that the risk for relapse of depression prozac and pregnancy is indeed very high during pregnancy," Cohen prozac and pregnancy says. "I worry about pregnant women who have a history of depression and who stumble across this CERHR prozac and pregnancy report. I worry that this huge effort on the part of prozac and pregnancy CERHR will actually not help with the clinical care of patients." Prozac is a Class B medication, meaning that it is recommended that it prozac and pregnancy be discontinued during pregnancy unless deemed medically necessary by a physician. A study in The New England Journal of Medicine reported that women taking Prozac during their pregnancies do not have a higher risk of miscarriage or major fetal abnormalities, but if taken during the third trimester they are prozac and pregnancy at a higher risk of perinatal complications such as premature prozac and pregnancy birth. A cohort studywhich followed up pre-school age children whose mothers had taken Prozac during pregnancy indicated, however, prozac and pregnancy that these children did not experience any adverse effects on their neurological development In contrast, it prozac and pregnancy was found, that untreated maternal depression can have an adverse effect on infant's and young children's cognitive, language, behavioral, and prozac and pregnancy perinatal risks. Obviously there are some risks associated with Prozac use during pregnancy, but the mother's mental well being is also a risk factor. Following childbirth, many mothers experience a condition called postpartum depression or "baby blues". Since this a period of high risk for depression under normal circumstances, women with chronic depression often choose to resume their medications during this time. There are many mothers, however, who choose to enjoy the prozac and pregnancy full motherhood experience prozac and pregnancy and breast feed. Not much is known about what levels of Prozac actually are found in breast milk so it prozac and pregnancy generally recommended that mothers not nurse their babies if they are talking Prozac. Perhaps the most difficult factor prozac and pregnancy in making a the decision whether to take antidepressant medications is the mother herself. During pregnancy, there is a strong impulse to do everything possible to have a healthy baby. However, the prozac and pregnancy mother often ignores her own prozac and pregnancy needs. A predisposition towards depression may complicate matters. The telltale signs of depression may creep up before the mother is even aware of them. Susan (not her real name) was determined to do everything possible to prozac and pregnancy ensure her baby's wellbeing. She ate right, exercised, didn't drink or smoke, never missed a doctor's appointment, and prozac and pregnancy stopped taking her antidepressants "just in case they might hurt the baby". During the seventh month of her pregnancy she began to think that maybe prozac and pregnancy her husband and baby would be better off without her. At that time, she says, "My thoughts made perfect sense. Keep in touch with your physician and let your friends and family know that they should monitor you for the signs of depression. If there's any doubt about your ability to handle being without your prozac and pregnancy medications, seek support prozac and pregnancy and if necessary resume taking your medication.
